SW Polk Fire District Residents (ASA 2) pay $75.00 and Polk County Portion of Amity Fire District (ASA 2) pay $95.00, $20.00 of amount goes to local Fire District, per benefit year for the entire household.

Membership benefits apply to SW Polk District residents and Polk County Portion residents served by SW Polk Fire District and are valid for the entire benefit period (1/1 – 12/31) if registering during the open enrollment.

If registering outside of open enrollment, benefits are valid after deposit of payment (plus one business day) through Dec 31st of the current benefit period.
